Add this to your userpage by pasting the code {{tlx|User was thanked}}, which, if you were [[User:Example]], would produce this:

To link to a thanks log for someone other than the page you're transcluding onto, set the first parameter to that name, e.g. {{tlx|User was thanked|Example}}.

To use the alternate style previously found at [[Template:User has thanked]], which was merged here and then changed into a different template, use {{tlm|User was thanked|style=2}}:
